Meet your physician

Sit down one-on-one. Bring your questions, your medication list, and any records you have. Come as you are.

The review

This is where it gets decided. Your physician reviews your health history against Georgia's qualifying conditions, honestly and thoroughly.

Leave with your plan

You'll walk out knowing exactly where you stand. If you qualify, you leave on the fastest path to your card.

Eligibility and certification are reviewed by the physician. Certification is not guaranteed; if you are not certified, the fee is refunded in full.

What happens after your visit.

Georgia's process has three parts, and you only handle two of them. Here is exactly what the state does, what we do, and what you do.

The doctor does this

Physician certification

If you qualify, your physician completes Georgia's Physician Certification and files it to the Medical Cannabis Patient Registry. There is no state form for you to fill out.

You do this

Upload your Georgia ID

Right after checkout you upload a photo of your Georgia driver's license or state ID. If the address on it is not current, add one proof of residency, such as a utility bill or pay stub in your name.

The state does this

Pay the fee, get your card

Once DPH verifies your certification, they email you a link to pay the $30 card fee plus a $3.75 portal fee, due within 60 days. Your card ships by UPS, signature required, in about 10 business days.

Georgia renamed the program the Medical Cannabis Patient Registry on July 1, 2026. Cards are valid for five years and the state requires an annual physician certification to keep them active. Questions about the registry itself go to the Georgia Medical Cannabis Program at (770) 909-2765, Monday to Friday, 8 AM to 4:30 PM.
